2、06NOTICEAccordingtotheLawnº7565,dated19December1986,theAeronauticalAccidentInvestigationandPreventionSystem–SIPAER–isresponsiblefortheplanning,guidance,coordinationandexecutionoftheactivitiesofinvestigationandpreventionofaeronauticalaccidents.TheelaborationofthisFinalReportwasconducted
3、takingintoaccountthecontributingfactorsandhypothesesraised.Thereportis,therefore,atechnicaldocumentwhichreflectstheresultobtainedbySIPAERregardingthecircumstancesthatcontributedormayhavecontributedtotriggerthisoccurrence.Thedocumentisnotfocusedonquantifyingthedegreeofcontributionofthed
4、ifferentfactors,includingtheindividual,psichosocialororganizationalvariablesthatconditionedthehumanperformance,andinteractedtocreateascenariofavorabletotheaccident.Theexclusiveobjectiveofthisworkistorecommendthestudyandtheadoptionofprovisionsofapreventativenature,andthedecisionastowhet
5、hertheyshouldbeappliedbelongstothePresident,Director,Chiefortheonecorrespondingtothehighestlevelinthehierarchyoftheorganizationtowhichtheyarebeingforwarded.ThisReportdoesnotresorttoanyproofproductionprocedureforthedeterminationofcivilorcriminalliability,andisinaccordancewithitem3.1,Ann
6、ex13ofthe1944ChicagoConvention,whichwasincorporatedintheBrazilianlegalsystembyvirtueoftheDecreenº21713,dated27August1946.Moreover,onemuststresstheimportanceofprotectingtheindividualsresponsibleforprovidinginformationrelativetotheoccurrenceofanaeronauticalaccident.TheutilizationofthisRe
